What You Need To Apply

  • 2 Forms of ID (Generally Driver’s License & Social Security Card)
  • Current License To Practice (RN, LPN, CNA, CMA)
  • BLS, ACLS & PALS Certifications (Must be American Heart Association)
  • Three Personal References
  • Complete Work History
  • Auto Insurance
  • Resume (Preferred)
  • Current TB Skin Test or Chest X-Ray (with Proof of Positive TB Skin Test)
  • Varicella Shot Record
  • MMR Shot Record (Applicable in Some Areas)
  • Hepatitis B Shot Record
  • Tdap Shot Record
  • Flu Shot Record (If In Season)
