- Current License to Practice (RN, LPN, CNA, CMA)
- Current CPR certification.
- Current TB skin test or chest X-ray (with proof of a positive TB skin test)
- MMR shot record (applicable in some areas).
- Hepatitis B shot record
- Resume (preferred)
- Three personal references
- All work history
